1. Introduction
This statement is made pursuant to Section 54(1), Part 6 of the Modern Slavery Act 2015 and sets out the steps taken by Evolutio Care Innovations Ltd (“Evolutio”, “the Company”, “we”, “us” or “our”) during the financial year ending 31 March 2025 to prevent modern slavery and human trafficking within our operations and supply chains.
Modern slavery is a crime and a violation of fundamental human rights. It takes various forms, including slavery, servitude, forced or compulsory labour and human trafficking. Evolutio has a zero-tolerance approach to all forms of modern slavery and is committed to acting ethically, transparently and with integrity in all business dealings.
As an independent healthcare provider operating in the United Kingdom, we recognise our responsibility to patients, service users, employees, suppliers and the wider community. We are committed to ensuring that modern slavery and human trafficking do not take place within our organisation or our supply chains.
We fully support the UK Government’s objective to eradicate modern slavery and to protect and support victims. We acknowledge our role in identifying risks, taking preventative action and responding appropriately where concerns arise.
2. Our Organisation and Supply Chains
Evolutio Care Innovations Ltd operates within the independent healthcare sector in the United Kingdom, providing specialist ophthalmology services.
Our supply chains primarily include UK-based suppliers providing:

We assess that our direct supply chain operates predominantly in lower-risk sectors within the UK. However, we recognise that modern slavery risks may still arise indirectly through subcontracting arrangements, global manufacturing of medical products, or complex multi-tier supply chains. We therefore remain vigilant and apply proportionate due diligence controls.

5. Risk Assessment and Risk Management
During the reporting period, we assessed the nature and extent of our exposure to modern slavery risks by:

Considering geographic risk exposure within extended supply chains
While the majority of our direct suppliers are UK-based and operate within regulated sectors, we recognise that risk may arise further down the supply chain, particularly where medical equipment and consumables are manufactured overseas.
We therefore continue to strengthen supplier engagement and awareness, taking a risk-based and proportionate approach.
We do not tolerate slavery or human trafficking in any part of our business. Where evidence of non-compliance is identified, we will require remediation and reserve the right to terminate contractual arrangements.
6. Training and Awareness
Evolutio requires relevant staff to complete modern slavery and safeguarding training, with periodic refresher training.
