How to refer
As a clinician you can refer any NHS CCG patient to Evolutio Ophthalmology for NHS funded care using any of the following methods:
GPs can:
1. Find us on ERS as “Ophthalmology Diagnostic & Treatment Centre – <TOWN> – Evolutio”
2. NHS email – LCHevolutio@nhs.net
Optometrists can:
1. eRefer via the NHSX approved electronic referral website at <insert link> where you will also receive the care outcome feedback for any appointment in our service
2. eERS via your local electronic referral system of choice and choose “Ophthalmology Diagnostic & Treatment Centre – TOWN – evolutio”

NCL NHSx programme
The NHSx EeRS programme, a new eye care service, commissioned by NHSX and supported by NHS London, designed to help improve referral pathways between primary care optometry and hospital eye services and promote an improved experience for patients being referred for ophthalmology care.
